  • Will you need childcare for any children in your care during VBS who are unable to participate in VBS? If so, how many and what are the children's ages?
  • I consent to the use of my image or voice in photographs, audio and/or video recordings taken during the course of the event for the purpose of promoting and marketing.
  • I consent to a background check for the safety of the children.
